Referral Department


General Information:

The Referral department is here to assist you with scheduling appointments to outside providers and to help guide or direct you with any questions or concerns you may have with your healthcare or provider.  Below is a list of tips you can reference about the referral process.


If you know you have an appointment coming up, please call us immediately so that we can get the process started.  We need at least 3 business days to process your referral.


If you’re calling for a referral, please note the date and time of appointment, provider’s name, name and location of facility and reason for referral.  Please leave a voicemail if we do not answer.


Remember, you need a new referral for every visit outside of the Stockbridge Munsee Health & Wellness Center.  This includes all follow-ups and any procedures.


A referral to an eye clinic is only needed from our department if it is related to an eye injury or medical condition such as glaucoma, diabetes, cataracts, etc.  Please see PRC for regular eye exams and glasses.


If our office schedules an appointment for you that you cannot attend, you may call and re-schedule the appointment yourself, just remember to call our office with the new date and time.


You need to be seen by one of our providers here at the Stockbridge Munsee Health & Wellness Center for any and all health concerns.  It will be determined at that visit if a referral is needed.
