Community Health Programs

Lifeline Program:

  • Lifeline, an emergency alert program is available to eligible member.
  • Referrals can be made for assessment of Lifeline to promote safety for elders in their homes.
  • ​Having a land line phone installed in the home is necessary for signal transmission.  
  • ​​An application must be made and approved prior to installation of the Lifeline which is a service in conjunction with Langlade Hospital.


Hospital Discharge Follow-ups:

  • Please call us and let us know when you are a family member are admitted to the hospital.  Please call or ask the hospital to call our Community Health Nurse 715-793-5014.  We will assess your discharge medication to be sure you understand what the doctor has ordered for you.
  • With your permission, we will work in conjunction with hospital discharge planner to make sure all necessary arrangements are made for when you come home.  


Medication Assistance:

  • Having trouble with not understanding your medications or not remembering to take them on time? One of our Registered Nurses will set up your medications in a pill box- making it easier to know when and how to take  your medications.
  • We will keep your prescriptions filled through the SMHWC Pharmacy and deliver them to your home and at that time your pill reminder box will be filled.


Loan Closet:

The SMHWC maintains a loan closet for Durable Medical Equipment consisting of some of these items:

  • Crutches, Shower Chair, Toilet Seat Riser, Commodes, Walkers, Canes, Grab Bars for Showers, and Wheelchairs.  


Immunization:

  • Annual influenza clinics
  • Individual Immunizations as necessary

Environmental Services-  light housekeeping (sweeping, dusting, vacuuming, mopping, clean bathroom, change bed linens, and food preparation) *No heavy lifting, rearranging furniture, washing outside windows, or outside work.
